REQUEST
Meeting room reservation must be submitted at least seven days before the requested date.
AFTER-HOUR USE REIMBURSEMENT
Meeting rooms may be used before or after regular hours of operation, strictly at the discretion of the YCFLD Administration. The YCFLD Administration may charge the requestor to reimburse Library District for after-hour personnel expenses at a flat rate of $25.00 per hour.
CERTIFICATE OF INSURANCE
Certificate of Insurance may be required by Groups or Organizations at the discretion of the YCFLD, in which case the Certificate of Insurance shall be provided to the YCFLD no later than 15 days prior to the herein described meeting or event.
APPROVAL
Permission to use the meeting rooms will be granted upon acceptance of the conditions for its use as expressed in the Meeting Room Policy 1.10. The User Affidavit below signifies that the requester received, understands, and agrees to the conditions contained therein
The YCFLD does not advocate or endorse the viewpoints expressed during any program or the viewpoints and policies of any group or organizations that uses library meeting rooms for an activity or program
USER AFFIDAVIT
I certify that I am duly authorized official representative of the Group or Organization making a reservation for use of the YCFLD meeting rooms. I have read the YCFLD Meeting Room Policy 1.10, fully understand the terms and conditions of use imposed by the Policy and this Agreement and agree, on behalf of the Group or Organization, to comply with all terms and conditions of use.
